Advanced Care Planning : This is a website which serves as a great resource for a process to think about, talk about and plan for life-threatening illness or en-of-life care. The Advanced Care Planning Guide is a 20 page document which helps guide you through the advanced care planning process. Two forms are included. First you will see the Durable Power of Attorney for Health Care. Then, you will see the Living Will. Both forms are very helpful to complete in order to insure you are getting the care you want. The time to complete these forms is after a conversation with family, friends and your health care provider.
